On September 7, 2021, the Board of Trustees took action to require that “all employees and students who come to campus are either fully vaccinated or undergo regular COVID-19 testing and produce proof of negative COVID-19 test results.”  Further, this action provided that all vaccinated employees by November 1, 2021, are required to provide proof of vaccination by uploading appropriate documentation utilizing the Citrus College mobile app. 

As we prepare for a substantial return of students and faculty to campus with the spring semester, it is important for us to ascertain how many of our employees are currently vaccinated and how many have already uploaded proof of vaccination.  The information from this brief survey will assist with planning the COVID-19 testing program for spring and assist with other COVID-19 protocols.

Per the current CDC definition, an individual is considered fully vaccinated two weeks after completing the second dose of a two-dose COVID-19 vaccine (e.g., Pfizer or Moderna) or two weeks after receiving a single dose of a one-dose vaccine (e.g., Johnson & Johnson).

If you indicate “decline to state,” you will be considered unvaccinated with respect to the COVID-19 protocols and testing requirement. Please complete this vaccination status survey no later than December 13, 2021.
